Your First Visit

At Round Rock Oral Surgery & Dental Implants, we’re committed to offering information and support to you at every step of your oral health journey.

During your first visit, you’ll have the opportunity to learn more about our team, meet your surgeon, and discuss treatment options.

Before you visit us for the first time, please fill out these registration forms to save time. If you have not, please arrive a few minutes early to fill out any necessary paperwork.

What to Bring

Please bring the following to your first appointment:

  • Referral slip and X-rays, if applicable
  • A list of current medications
  • Medical/dental insurance information
  • Valid identification (e.g., driver's license)
  • Form of payment

Please note: If you are under 18 years old, a parent or legal guardian must accompany you to your consultation appointment.

What to Expect

Your first appointment with us is a chance for Dr. Weber and you to get to know each other. He will review your health history, listen to any concerns, perform an oral exam, and discuss what you hope to gain from oral surgery and treatment options available.

While some treatments may be completed at your first visit, a majority of procedures require a second appointment. If this is the case, we will provide you with detailed instructions and information so you are prepared for your scheduled surgery date.

3D Imaging

Our practice utilizes cutting-edge technology with a fully digital setup, featuring state-of-the-art CBCT scanning and imaging, digital X-rays, digital impressions, intraoral scanners, and guided implant placement.

Financial & Insurance Information

For your convenience, we accept the following forms of payment:

  • Financing
  • CareCredit

Our commitment is to provide exceptional care at a reasonable cost, and we kindly request payment at the time of service unless prior arrangements have been made. If needed, we can arrange a monthly payment plan, but these arrangements must be made prior to the procedure.

We are dedicated to providing reasonably priced, exceptional care. Therefore, unless other arrangements have been made we require payment at the time of service.
